Home Health - Therapy
Making the transition from nursing home, rehab, or the hospital to home can be challenging. We are committed to helping make the transition as smooth as possible and ensuring that all of your needs are met.
Physical Therapy - Our physical therapists will evaluate each patient in their home and develop an individualized program to address the patient's specific needs. A great advantage of home physical therapy is working in the comfort of your home. Training you in this familiar environment allows you to overcome everyday obstacles. Physical therapy can help improve a patient's strength, balance, and ability to transfer. We can also make recommendations on ways to improve the safety of your home and reduce the risks for falls. In addition, we can provide education to the caregivers to help them reduce risk of injury to themselves and the patient. Caregiver education will also include demonstrations of the home exercise program. Our goal is to increase the independence of the patient, increase the ability of the caregivers to provide assistance safely, and provide a more functional home environment.
Occupational Therapy - Our occupational therapists will evaluate the home environment and address adaptive activities and safety needs.
Speech Therapy - Our speech therapists focus on communication barriers and oral motor defects causing problems with chewing or swallowing.
