About Continuing Care
Continuing Care is a full service agency that has been serving the greater Shenandoah Valley since 1988. We are unique in that we specialize in both home health and personal care services, providing continuity of care. Continuity of care allows our skilled professionals to work together for a more holistic approach. The client and family can have all their needs met by one agency. Our services are designed to complement each other allowing clients to live comfortably at home.
Our goal is to help you remain as independent as possible, in your home. Whether you have been recently discharged from the hospital or just had surgery, are elderly, ill or disabled, Continuing Care can provide the variety of services you need in a professional and confidential manner. Our highly skilled clinical staff will assess your needs and develop a customized plan of treatment to assist you in maintaining, or gaining back, your independence. All of the care we provide is under the supervision and guidance of registered nurses, regardless of the level of care our clients initially need. We know that over time a client's condition may change. We strive to identify these changes early and assist with providing the level of care necessary to prevent an unnecessary hospital admission and protect the safety and well-being of our clients. The best way to keep our clients safe and in the comfort of their own home is to recognize that small interventions caught early can prevent a significant deterioration in health or well-being causing them to lose the independence they value so dearly.
