We are committed to making the transition from a nursing home, rehab, or the hospital to you home as smooth as possible. We offer Skilled Nursing, Home Health Aides, Occupational Therapy, Physical Therapy, and Speech Therapy to ensure all of your home health needs are met.
To be eligible to receive home health services, you must be homebound due to illness, injury, or after hospitalization. Being homebound means that it is a taxing effort to leave home and that you are unable to do it alone.
Skilled Nursing
Continuing Care provides skilled nursing services to our clients under the care and direction of your physician. A Registered Nurse will provide a thorough assessment of health and functional abilities, a review of medications, a coordination of physician ordered services, and a comprehensive care plan, based on total evaluation of needs.
