Home Health

We are committed to making the transition from a nursing home, rehab, or the hospital to you home as smooth as possible. We offer Skilled Nursing, Home Health Aides, Occupational Therapy, Physical Therapy, and Speech Therapy to ensure all of your home health needs are met.

To be eligible to receive home health services, you must be homebound due to illness, injury, or after hospitalization. Being homebound means that it is a taxing effort to leave home and that you are unable to do it alone. 

Skilled Nursing
Continuing Care provides skilled nursing services to our clients under the care and direction of your physician. A Registered Nurse will provide a thorough assessment of health and functional abilities, a review of medications, a coordination of physician ordered services, and a comprehensive care plan, based on total evaluation of needs. ​

Home Health Aides
Specially trained aides are available to assist with personal hygiene, nutritional support, and environmental maintenance, under the supervision of a clinician. ​
Therapy Services
Home health therapy allows you to receive treatment in your real world environment and address the challenges that are unique to you and your home. Choosing home health therapy services may allow you to return home from an inpatient facility earlier, help you remain at home longer, and maintain your independence as long as possible. Our goal is to increase your independence, increase the ability of each caregiver to provide assistance safely, and create a more functional home environment. Our skilled therapists will provide a comprehensive assessment of functional abilities and home safety, working in conjunction with your local physician.

​Physical Therapy
Physical Therapist's primary goal is to develop, maintain, and restore maximum movement and functional ability throughout the client's life. This service is especially needed in instances where mobility is hindered by the process of aging or due to injury or disease.
Physical Therapy can help you:

Occupational Therapy

Clients may learn how to conserve energy, improve their basic motor functions and reasoning abilities , and how to compensate for permanent lack of function. The goal of occupational therapy is to help clients lead independent, satisfying, and productive lives.

  • Bath and perform personal hygiene
Speech Therapy
Speech Therapists address client's speech, swallowing, and language difficulties. Depending on the nature and severity of the disorder, treatments may include physical strengthening exercises, instructive or repetitive practice, and the use of audio-visual aids.
