Continuing Care provides personal care services allowing our clients to maintain their lifestyle in the comfort and familiarity of their homes. Our highly skilled clinical staff will work with you to determine the level of care necessary to improve your quality of life. We will assess your needs and develop a customized plan to assist you with your activities of daily living.

Our Nursing Assistants are all supervised by a Registered Nurse to ensure your needs are being met. Registered Nurses are available 24 hours/day to help answer your questions. If you are Medicaid eligible, you may qualify for services that are fully covered through a waiver program. We also accept most private insurances.
Respite Care Services
Respite Care is also available for those receiving Personal Care. If there is a primary caregiver that assists you on a regular basis, you may qualify. The intention of respite care is to provide relief to your caregiver. This service can be used for caregivers to run errands, go shopping, take care of personal business, or get some much needed rest.
Private Care Services
Continuing Care can provide private care services by trained aides that are tailored to meet your needs. These services can be provided in hospitals, nursing homes, assisted living facilities, or in the comfort of your home. Our services range from 2 to 24 hours a day. Most often, private care is paid for by the client or his/her family. Your insurance company may cover this and we will be glad to assist you in filing for preauthorization.
